Stabilising crestal bone in thin tissue patients

A new study from Lithuania has examined the use of subcrestal implants and 2mm healing abutments to reduce crestal bone loss in patients with thin soft tissue.
It has been posited that vertically thin soft tissue may be related to crestal bone loss. A number of methods have been proposed to counteract this and increase soft tissue thickness. One proposed method is subcrestal placement of an implant followed by bone remodelling; another is placing an implant at bone level with a 2mm healing abutment.
Based on these proposed methods the study had several aims: to determine the bone loss in the subcrestal and epicrestal implants; to register the level of bone remodelling in subcrestal implants and finally to determine the increase in soft tissue for the epicrestal implants.
24 patients were involved in the study. The cohort was divided into two groups, and subcrestal and epicrestal implants were placed in equal measure. At the second stage of surgery all implants received 4mm healing screws and screw-retained single crowns. Radiological assessment was carried out at this stage and again one year post-surgery.
Slight crestal bone loss was observed in both subcrestal and epicrestal implants which suggests both methods could be used to limit bone loss. Bone remodelling was found to be larger for subcrestal implants when the results from the two groups were compared. Finally, the authors note that tissue thickness was seen to increase following the placement of an epicrestal implant with a 2mm healing abutment.
